The evidence arrives faster than anyone has time to read it.
The problem
A decade ago there were a handful of options in advanced prostate cancer, and one fairly obvious order to give them in. There are now dozens of agents, used alone and in combination, across several lines of therapy. The number of possible sequences runs into the thousands. Only a small fraction of them have ever been studied.
And the reading grows every month. Trials report, then report again with longer follow-up, then again with an analysis that changes what the first readout meant. Two or three major congresses a year add to the pile. Nothing is ever finished. It is only superseded.
Meanwhile the time has gone the other way. Clinics are fuller, lists are longer, and the hours left for reading are fewer than they were. Keeping up stopped being a question of diligence some years ago. It became a question of arithmetic — and the arithmetic does not work.
What it does
- Puts it in one place. Every trial that bears on the decision, held together and kept current, organised around a patient rather than around a publication date.
- Answers in seconds what would take an evening. Ask what the evidence supports for the person in front of you, and have the answer while they are still in the room.
- Stays current so you do not have to. When a trial reports, it goes in. Nothing depends on you having seen it.
- Separates what is settled from what is not, so the reading time you do have goes to the questions that are still genuinely open.
